Case Management
• Case management is a collaborative process where a medically qualified person, such as a nurse, works with a patient, his or her family, the treating physicians and community resources to ensure the optimum outcome for the patient. The case manager helps identify and coordinate the available resources, and works as a liaison between the patient and claim manager throughout the continuum of care. In short, the case manager often sees the forest, while individual caregivers are focusing on their respective trees.
